Setting up an appointment booking agent for a clinic

A front desk agent that answers every call, books and moves appointments and never gives medical advice. The setup, the tools and the guardrails.

Clinics miss calls when the front desk is busy with patients in front of them, and missed calls are missed appointments. An agent on the clinic's number can answer every call, handle the routine ones end to end and hand the rest to a person.

What the agent should handle

Keep the scope to what the front desk does on the phone every day:

  • Book a new appointment with a doctor or speciality.
  • Move or cancel an existing appointment.
  • Answer timing and location questions (from a short knowledge base).
  • Take a message for anything else.

Everything clinical goes to a person.

Connect it to your calendar

The agent needs to see real availability. Give it two tools that call your booking system's API:

  • check_availability(doctor, date) returns open slots.
  • book_appointment(doctor, slot, patient_name, phone) books one and returns a confirmation.

Tools run during the call, so the agent offers only slots that actually exist. If you don't have an API, start with a webhook to a sheet and a person who confirms bookings, then automate it later.

The guardrails that matter

Write these into the prompt plainly:

  • Never give medical advice, diagnoses or dosages.
  • If the caller describes chest pain, breathing trouble, heavy bleeding or anything that sounds urgent, tell them to call 112 or go to the nearest emergency room, and transfer the call to the clinic if transfers are set up.
  • Don't share any patient's details with a caller, including whether someone has an appointment.

Turn on transfers with the front desk's number as the target, so the agent can hand over with a short summary instead of saying "please call back".

Confirm and remind

After booking, send an SMS confirmation from your system (the booking tool can trigger it). A day before the appointment, an outbound reminder call can confirm attendance and offer to reschedule, which frees slots for other patients when people can't make it.

Language

Patients call in Hindi, English, Hinglish and the local language. Set the agent to reply in the caller's language and choose a voice that says doctors' names and locality names correctly. Add those names as transcriber keyterms.

What to measure

Look at answered calls (should be close to all of them), bookings made by the agent, transfers, and the reasons calls were transferred. The transfer reasons tell you what to teach the agent next.
